Requirements to Start Physical Therapy

Your first visit will be a comprehensive evaluation of movement dysfunction, consisting of joint restriction, neural mobility, and muscle extensibility.

As written by the American Physical Therapy Association, Maryland is an Unrestricted, Direct Access state. There are no restrictions or limitations whatsoever for treatment outside a referral from a physician.

There are individuals who prefer to have a referral before attending physical therapy.  Ask your local primary care physician or orthopedist about how physical therapy can reduce or eliminate your pain or discomfort.  For those who have experienced the benefits of physical therapy and would like to schedule an appointment, refer to the “Required Documents” forms listed below.  If you are more comfortable scheduling an appointment via telephone, or if you have any questions or concerns, visit the ‘Contact’ page for more information.

Prior to your first visit we ask that you please complete the Patient Intake Form.
